This brief questionnaire, will take only a few moments, but will contribute valuable information to a report by Liverpool Wheelchair User Group regarding the provision of wheelchair services for children and young people in Liverpool. The report will be shared with Liverpool Wheelchair Service and Liverpool Community Health.
